Connecticut ranks among the top states in measures of the health of its citizens, yet a closer look reveals astonishing inequities in healthcare outcomes and health status. Connecticut's State Innovation Model (SIM) Initiative brings payers, providers and community stakeholders together to improve health system performance, increase quality of care and decrease costs while addressing healthcare inequities. The Connecticut Health Council's March 31 program will explore how Connecticut health leaders are improving community health through the SIM Initiative.
